Strange behavior with intro image insertion

I've created a module that inserts the article intro image into the header area for each page, so the header can be customized. There's already a background image in place in case the article has no intro image. This was working when the site was initially created. However, this past week we made several changes to intro images and noticed that the new intro images are only visible on each page if you're logged-in. When not logged in, the intro image from the first page you view is then shown on every other page you visit.

So if you visit the About page first, then navigate to another page, the intro image stays the same, even though they have different images. I'm sharing a few of the links below:

That sounds like a caching issue. Switch off caching on the module.

That seems to have resolved that issue! One more question on this setup - the header area is flashing the default background image before it loads the intro image. Would it be possible to use the if,then setup so that all images are loaded through the module? So it loads the intro image as the background, but if there's no intro image set, it loads a default image instead?

Yes, something like:
{if image-intro}[image-intro:url]{else}your/backup/image.jpg{/if}
