I've used your sourcerer on a few different project but recently had a client ask me to put a script in that runs a JS button pop-up. I've tried a number of way, but can't get the script to run correctly.
What do I need to do to make this button appear on the Joomla 3 site?

What exactly have you placed (including the {source} tags)?
And where? In an article?

I tried using the pro version all ways on this, but I'm missing something. If you look at the sample site, you will see the script in the content, but that JS needs the head information to function. I'm not sure what I'm missing to make this work like the sample.
I've added the script back in a custom module to the referenced page, but no "button".

So Sourcerer is working fine. It is placing the scripts you tell it to, as you can see in the html output of your site.
The script is throwing an error. That is down to you not including the script that has the SaiaMTMButton class.
This is all outside the responsibility of Sourcerer.
