I love auto mode but wondering if there is a way to set it to show as "after X paragraphs" or even just after the start of the main text / full article text. Aesthetically this would look better I think.

There is no such option in Quick Index itself.
You could possibly use ReReplacer Pro to insert the {index} tag after x paragraphs or before/after something specific in your article content.

Is this something that I can do manually by editing the files? Would this be difficult for the developers to add as a feature in an upcoming version?

There are no plans to add this to Quick Index.
Not sure if you can edit the Quick Index files to make it work.
