Been using rereplacer recently to change the flags from the language switcher module. Right after upgrading from 12.4.0 to 12.5.1 the flags disappeared and the link was being displayed instead of the 'new' image.
I reverted back to 12.4.0 and it's working now.

Not sure why that would happen.
Can you try changing the ordering of the system plugins? See if that helps.

Did that and.. slightly better: I have the alt text only appearing now. The HTML code is this:
<img title="ខ្មែរ" data-src="<!-- RR_REPLACE_SPLITTER -->/images/km_kh.png<!-- RR_REPLACE_SPLITTER -->" alt="ខ្មែរ" class=" lazyloaded" src="<!-- RR_REPLACE_SPLITTER -->/images/km_kh.png<!-- RR_REPLACE_SPLITTER -->">
There are 2 plugins: the libraries and the rereplacer one. They were at the top of system plugins, placing them at the bottom did that.
And FF developer tools is telling me "could not load the image" when I mouse over the image link.
Switching back to v12.4.0 fixes the problem.

That's not good. Looks like the last part of the process in the plugin is not getting executed.
Can you give me (super) admin and (s)ftp access so I can take a look?
And tell me on what url I can see/reproduce the issue.
You can use the 'Confidential information' button in the forum editor to hide sensitive information.
